Revised text to state that it is unlikely that response to induction chemotherapy, as judged by anatomic imaging, correlates with the likelihood of survival in patients with rhabdomyosarcoma, on the basis of the Intergroup Rhabdomyosarcoma Study Group, Children's Oncology Group, and International Society of Pediatric Oncology studies that found no association (cited Vaarwerk et al. as reference 63 and level of evidence 3iiiA).
Added text about the results of a study on the patterns of failure in 11 of 66 children with nonmetastatic rhabdomyosarcoma who were treated with proton radiation therapy (cited Vern-Gross et al. as reference 25).
